A quarry worker died after becoming entangled in rotating machinery at a South Canterbury quarry. The operator was working alone at the time.
The exact time and circumstances that led to this fatality are unknown. He was only discovered after a neighbour noticed the quarry machines were still operating much later in the day than normal and went to investigate.
WorkSafe NZ advice
WorkSafe NZ is investigating this incident and advises that the sector needs to ensure all machinery is properly guarded, and fences restricting access to machinery are properly secured. Perimeter guarding does not negate the need for individual guards on dangerous moving parts.
Operators should guard machinery to meet the AS/NZS 4024 Safety of machinery series, as well as other relevant standards.
